Your coaching intake form template
Copy the questions below or download them as a text file. Adapt them before adding them to your form.
- Your name
- Your email address
- What would you like to work towards through coaching?
- What prompted you to enquire now?
- What kind of support are you looking for?
- Do you prefer online or in-person sessions?
- When would you like to begin?
- Is there anything else you would like to ask before we speak? (Optional)
Keep the first application focused
A first enquiry should help you understand the person’s broad goal and whether a conversation makes sense. Keep questions short, explain why you are asking and leave detailed onboarding for later. A smaller form is also easier for someone to finish on their phone.
- Contact details for a reply
- The goal or topic they want to work on
- Their preferred coaching format and timing

Apply for coachingAdapt the questions to your coaching
A life coach may ask what prompted the enquiry. A business coach may ask about the decision the person is working through. A personal trainer can ask about broad goals and the kind of service they are interested in, keeping health screening separate from this first-contact template.
Keep each answer with the enquiry
Use a contact or application form on your Orbits website to collect the details you need. Submissions arrive in the Leads list with the person’s answers, giving you a place to prepare for the next conversation. Review your published form before sharing the page.
Coaching Intake Form: your questions answered
Is this coaching intake form template free?
Yes. You can copy the questions or download the text without signing up. Adapt them for the way you work and the tool you use to collect enquiries.
Is an intake form the same as a coaching agreement?
No. This template helps with a first enquiry. It does not replace your coaching agreement, service terms, consent process or any specialist assessment required for your work.
How many questions should I ask?
Ask enough to decide the next useful step. Name, a contact method and a few focused questions about goals and preferences are a reasonable starting point. Remove questions you do not need for the first conversation.
Can I use this for personal training applications?
You can adapt the broad goal and service-preference questions. Keep health information and screening in a separate process appropriate to your professional responsibilities.
Can I start free?
Yes. Start with a free Orbits workspace and try the website, lead capture and content tools. You can review your available usage and upgrade options in the app.
